We are CDC Full Plus Recognized Diabetes Prevention Provider and the first Umbrella HUB in Baltimore City and County. Expanding access to diabetes prevention in underserved communities with local organizations helps to delay and reduce type 2 diabetes diagnosis.  

Designed specifically for people 65 years or older with a prediabetes diagnosis. Program is a Medicare and Medicare Advantage covered program for beneficiaries with prediabetes.

Supporting pregnant moms diagnosed or at risk for gestational diabetes can enroll in a six week healthy lifestyle program to reduce adverse maternal risks.
