Community Health Initiatives

We are a CDC Full Plus Recognized Diabetes Prevention Provider and the first Umbrella HUB in Baltimore City and County. By partnering with local organizations, we are expanding access to diabetes prevention and healthy lifestyle programs in underserved communities, helping to delay and reduce the diagnosis of type 2 diabetes. Additionally, our initiatives support maternal health programs, emphasizing the importance of health for all members of the community.

Designed specifically for people 65 years or older with a prediabetes diagnosis, this diabetes prevention program is covered by Medicare and Medicare Advantage for beneficiaries. It is one of the healthy lifestyle programs that aim to support individuals in managing their health effectively.

Pregnant moms diagnosed with or at risk for gestational diabetes can enroll in a six-week healthy lifestyle program, part of a maternal health program, designed to aid in diabetes prevention and reduce adverse maternal risks.
